The nitty gritty - Price Right now I am working on a sliding scale of $120/$90/$60. This is based on what you feel you can afford at the moment, times are hard (and weird!). If the low end still makes you count the last bones in your bucket then let me know, we can figure out something and I’m surely not going to turn you away.

Disclosure and Consent

This form is an outline and understanding of our client/practitioner relationship. After reading this you can then make an informed decision as to whether you would like a consultation with me regarding your health history, concerns and goals. 

It is my role as an herbalist to provide you with information that will educate and empower you to understand the complexities of your personal health story and with that understanding we can work together to reach your health goals. 

I do not diagnose, prescribe or treat disease. My services are strictly educational. I provide you, the client, with the tools to help you support your well being and make educated decisions about your health care. The herbal protocols, supplements, vitamins and/or food/ nutritional diets that I recommend are not meant to replace medical diagnosis or treatments. All client records are confidential and will only be discussed with you, the client, unless authorized for release. 

Client Rights & Responsibilities-

It is your responsibility as the client to inform me, the practitioner, of all the aspects of your health, past and present and also to inform me of any changes that may occur. It is also your responsibility to immediately notify me, if you are experiencing any pain, discomfort or possible adverse effects. It is your responsibility to inform me if you are or believe you might be pregnant, are trying to get pregnant or are breastfeeding. 

You have the right to respectful and courteous care. You acknowledge that your participation in this herbal consolation is voluntary and that you are free to discontinue service at any time. 

Herb Safety-

Stay educated, it is YOUR health. Throughout history and into modern research, the herbs that are most often used have an exceptional safety record. Herb and drug interactions & allergies are rare but possible. It is your responsibility to fully disclose any medications, supplements, vitamins and/or herbs that you are currently taking so that I can offer you complete and well informed advice. Please inform me of any concerning reactions.
